Heart Failure Management — Current Guidelines Summary

Key updates from the ACC/AHA heart failure guidelines.

Stage A (at risk): Screen and modify risk factors — HTN, DM, obesity. Stage B (pre-HF): Echo-confirmed structural disease. Start RAAS inhibitor if EF <40%. Stage C (symptomatic HF): Guideline-directed medical therapy (GDMT) — ARNI, BB, MRA, SGLT2i. Stage D (advanced HF): Refer for advanced therapies — LVAD or transplant evaluation.

COPD Severity Classification Quick Reference

Current GOLD staging and spirometry thresholds.

GOLD 1 (Mild): FEV1 ≥80% predicted. Often asymptomatic. GOLD 2 (Moderate): 50% ≤ FEV1 <80%. Dyspnea on exertion. GOLD 3 (Severe): 30% ≤ FEV1 <50%. Frequent exacerbations. GOLD 4 (Very Severe): FEV1 <30%. Significant quality of life impact.

Week 1–2: PT daily, pain management, DVT prophylaxis. Week 3–6: Progressive weight bearing. OT for ADLs. Week 6–12: Strengthening. Stair negotiation. Return to driving. Month 3+: Full activity clearance. Follow-up imaging if indicated.

Blood pressure: every visit; confirm elevated readings out of office. Lipids: every 4–6 years for average risk; annually if on statin therapy. Colorectal: begin at 45 — colonoscopy every 10 years or FIT annually. Breast: mammography every 1–2 years from 40 per patient preference and risk. Diabetes: A1c or fasting glucose every 3 years from 35, sooner with BMI ≥25 + risk factors.
